Association of Betel Quid Chewing with Oral Complaint and Oral Health Status in Elderly Thai

Abstract
Objective: This study was conducted to clarify the effects of chewing a quid containing betel nut and tobacco on self-reported oral health problems and oral health status in community-dwelling elderly Thai.
Methods: The subjects were 428 subjects (93 chewers and 335 non-chewers with a mean age of 68.6±5.7 years) living in Phitsanulok, Thailand. Information on their sociodemographics, general health, health behaviors and self-reported oral health problems was obtained by a questionnaire. Oral health status (teeth, oral mucosa and periodontal status) were clinically examined. Their flow rates of unstimulated saliva were also assessed. Data were analyzed using Chi-square test and ANCOVA.
Results: For self-reported oral health problems, the incidence of bleeding gums was significantly higher in quid-chewers than non-chewers. The lower incidence of bad odor, taste disturbance and appetite loss were found in quid-chewers than the non-chewers group (p＜0.05). For clinical examination, gingival bleeding, periodontal pockets had a higher incidence in quid-chewers than in non-chewers (p＜0.05). The mean number of decayed teeth in quid-chewers was lower than in non-chewers group (p＜0.05).
Conclusion: Betel quid chewing had higher risk for various oral complaint and oral health status. The present data indicate that chewing quid comprising betel nut and tobacco had effects on dental caries, periodontal tissues and incidence of oral lesions. The results from this study could be used as base-line information for health authorities and dental professionals for planning strategies for oral health treatment, prevention and promotion among the elderly Thai, especially aiming at discouraging from chewing betel quid.
